This Client Feedback Form is a quick and simple 10 question form that can be used in a face to face interview or via phone call with your client. It has a check-the-box rating system ranging from Excellent to Bad and a variety of questions designed to give the user a comprehensive overview of your client’s satisfaction with the services that your firm has provided. Questions touch all the key points (Scope, Budget, Schedule, Quality, and Communication) that should be of interest to you and your client.

 It is helpful for survey firms to perform periodic check-ups on your clients/projects/project managers. A cornerstone piece to your internal quality control/quality assurance process, the Client Feedback form is designed to provide comprehensive feedback for many aspects of a project. This form can also be used to build references for future RFPs the firm responds to while also helping to retain current clients for future projects/repeat business. This form should be used by an interviewer that is not involved with the project so as to facilitate honest feedback, such as your Business Development person, another client manager not on the current project or firm executive.
